No-Show / Late Cancellation Policy

At Nutrition Refocused our goal is to provide quality nutritional care to all our patients in a timely manner. No-shows, late arrivals, and late cancellations inconvenience not only our providers, but our other clients as well.

 

We understand that things happen and can change your schedule at the last minute. When you book your appointment, you are holding a space on our calendar that is no longer available to our other clients. If you are unable to keep your appointment, please contact us immediately so we may reschedule you.

 

As a courtesy to our office as well as to your fellow clients who are waiting to schedule with the Dietitian, we require a 24 hours notice for any cancellations or changes to your appointment. You may contact us at (719) 428-5309 to cancel or reschedule your appointment. We do not accept email cancellations.

 

Clients who do not cancel or reschedule their appointment with at least 24 hours notice, may be charged a            “no-show/cancellation” fee of $75.00. This “no-show fee” is not reimbursable by your insurance company. You will be billed directly for it. 

 

To secure your appointment we require a valid credit card number to hold your appointment. Your appointment time is reserved for you, however, it is not fully booked in our system until a valid credit card is on file.  You will not be charged until the time of your appointment.

You understand and acknowledge the “no-show” policy of Nutrition Refocused LLC and agree to provide a credit card number, which may be charged $75.00 for any no-show or late cancellation of a scheduled appointment. I understand that I must cancel or reschedule any appointment at least 24 hours in advance via phone in order to avoid a potential no-show/late cancellation fee to the credit card provided.
